Pubjoe: Sonic lets you look up and down, but it moves the camera appropriately by itself as well. I'm wondering if it's the earliest occurrence of this.

I just noticed that Wonderboy kind of does this. It's not a free-scroller like Sonic, so again, not really what you're looking for, but it does intentionally adjust the player's viewpoint away from the norm. As a rule, you always push the screen horizontally from one third in. The scrolling path occasionally includes vertical slopes but regardless, it is always pushed into view by the players horizontal progress.

What's quite interesting is that the 'camera' path sometimes shifts vertically for seemingly no reason other than to make things more interesting. It doesn't necessarily optimise your viewpoint like with Sonic, but it does show an early stage where designers had, and used, the capability to do this.
