Seizure warning sticker.
Maybe someone here who speaks japanese could write the text down? with the japanese kanji's available it would be really easy to rebuild the sticker ;)

I've included a romaji translation (not my favorite for language-barrier reasons) but it's accurate. In case you computer can't type in Japanese I've also included the transcript in it's original kana form. At the very least you should be able to copy/paste it into Photoshop as a font layer. Here's a link for .ttf files which contain the caution symbol as well. http://www.dafont.com/font.php?file=warning_tables"

Awesome, thanks a lot for the translation Zensouken!

But you forgot the little letters above/between those five lines.
I've tried to copy/paste the letters from your text as much as possible and then used this site i've found today, for the rest of the letters. Maybe you could check if they're correct?

LINE 1> きゃく また こさま かこ ひかりしげき

LINE 3> ばあい きき りよう じゅうぶん ちゅうい
LINE 4> ちゅう きぶん わる ばあい ちゅうだん
LINE 5> きゅうけい と

I'm almost finished just need to adjust the caution symbol a bit and try to find a font that matches the original one better (but tbh i doubt i will find anything closer):

It looks like the height is about the same size as the ¥100 Sticker but i could be mistaken.

The smaller kana is called furigana. It's used for teaching people the pronunciation of more advanced kanji they might not know how to read. Well your image looks spot on, the furigana is correct. I can see that you are trying hard to get it as accurate as possible so I would say that the second kanji's furigana, "mata" is a little too far to the right. In general you want to center all the furigana as much as possible.

So if you happen to have MS Word, there is something called the 'Phonetic Guide' which will add the furigana to the Kanji and positioned correctly. Here's a link to understanding how to use it well.
http://thejapanesepage.com/fun/furigana_in_word" Hopefully you can export the kanji with furigana in tact to Photoshop. Or at least it will show you exactly where to position the furigana correctly.

Regarding the caution font, I think it looks good. You may have to search high and low to find one more accurate since I'm assuming the original caution font may have come from a Japanese-type wingdings or symbol set. But I think it looks damn fine as it is right now.

Good luck, I'm looking forward to seeing this completed. :D
