I've included a romaji translation (not my favorite for language-barrier reasons) but it's accurate. In case you computer can't type in Japanese I've also included the transcript in it's original kana form. At the very least you should be able to copy/paste it into Photoshop as a font layer. I've tried to copy/paste the letters from your text as much as possible and then used this site i've found today, for the rest of the letters. Maybe you could check if they're correct?

The smaller kana is called furigana. It's used for teaching people the pronunciation of more advanced kanji they might not know how to read. Well your image looks spot on, the furigana is correct. I can see that you are trying hard to get it as accurate as possible so I would say that the second kanji's furigana, "mata" is a little too far to the right. So if you happen to have MS Word, there is something called the 'Phonetic Guide' which will add the furigana to the Kanji and positioned correctly. Here's a link to understanding how to use it well.

Regarding the caution font, I think it looks good. You may have to search high and low to find one more accurate since I'm assuming the original caution font may have come from a Japanese-type wingdings or symbol set. But I think it looks damn fine as it is right now.
